What to expect at your first visit

In preparation for your first appointment, we request that you complete our PATIENT FORMS that will be emailed to you once your appointment is scheduled online or through our office. This paperwork provides us with your health history and information on your condition.  If you are scheduling for regular chiropractic care, please fill out the Adult New Patient Application.  If you are currently pregnant, please fill out the Pregnancy New Patient Application.  If the new patient is a child 14 years of age and younger, please fill out the Pediatric New Patient Application.  For pediatric patients under 18, they must be accompanied by a parent or legal guardian.   If you are unable to make it for your appointment, a 24 hour cancellation is required.  Please see our Clinic Policies page for details on canceling appointments.

If you are unable to complete the paperwork prior to your appointment, you will need to arrive 15 minutes prior to your scheduled appointment time to fill this out.  

Next, you will meet with Dr. Rhae for a history and examination of your condition, and expectations to determine how chiropractic care can meet your goals.  At the conclusion of this initial appointment, if it is determined that chiropractic care is appropriate, you can choose to experience your first treatment. This may include spinal adjustments, corrective exercises, and/or soft tissue mobilization. Treatment is interactive, so you can express concerns about the different styles of treatment anytime and nothing will be performed without your consent.
